Abstract

The paper presents the results of the analysis of the "News Divine" stampede incident which took place on 20 th June 2008. The fatal incident occurred when the police raid a packed nightclub to check reports that drugs and alcohol were being sold to underage. The approach has been the application of the Management Oversight Risk Tree (MORT) technique. The MORT is intended to look at various organizations failures that have led to the incident. The MORT consists of a large tree with several branches that are required to look at when assessing the incident. Some preliminary results of the analysis of what happened during the undesirable event have been presented. More work is needed in order to identify any management elements on the why branch of the MORT tree that contributed to the particular problems identified in the present analysis. Moreover, other accident analysis approaches may be used for the present case. It is hoped that by conducting such analysis lessons can be learnt so that similar events can be prevented in the future.
